NEW COMPUTER SYSTEM BEING INSTALLED MAY 2013Due to the installation of a new computer system for the surgery we will have no computers working from Monday 13th May to Thursday 16th May. Unfortunately this will cause some unavoidable disruption to our usual services. Please note the following: Please expect to wait longer than usual for a routine appointment as there will be fewer routine appointments available than usual for a limited period during the downtime and for a few days following. If possible please avoid calling between 13th - 16th May to make any routine appointments. The number of weeks appointments can be booked in advance will be reduced until our new system is installed. We will be unable to issue repeat prescriptions from 13th - 16th May. Please order any repeat medication that you may require during that time by the beginning of May and please allow longer than usual for any requests submitted after 16th May. We may be a little slower than normal in dealing with phone calls and other requests whilst we get used to the new system. Please bear with us during this time and we will do our best to get our normal service up and running as quickly as possible. If you have any queries or conerns about this please contact our Practice Manager, Dominique Pearson-Smith, or our IT Manager, Christine Pow at the Surgery. An Introduction To The Hawthornden SurgeryThis practice was founded by Dr Francis Gibbs in 1895 but he was to die at an early age in 1911. He was succeeded by Dr Selborne Bailey who was to remain with the practice from 1911 until 1968! In 1923 he amalgamated with the other practice in Bourne End; Dr Vaudrey Mercer joined the practice in 1934 and in 1943 Dr Bailey's son Alison entered the practice; Dr Mercer moved to Cookham in 1946. Dr Flanagan came as an assistant in 1954 and Dr Lloyd Parry joined as a partner in 1965 followed by Dr Fergusson in 1967. Dr Alison "Joe" Bailey retired from the practice in 1986, by which time he and his father had completed a total of 100 years of general practice. Sadly Dr Bailey died in November 1997. Dr Sarah Buxton joined as a partner in August 1992 and Dr Michael Wolfin in March 1996. He replaced Dr Lloyd Parry who retired in March 1996 after more than 30 years as a partner in this practice. Dr Tanveer Hussain joined the practice in January 1997 to be followed by Dr Peter Newman who joined the practice in May 2000 following the retirement in April 2000 of Dr Fergusson who had by then completed more than 32 years as a partner here. Our newest partner is Dr Sarah Abbas who joined us in February 2012 as a job-sharing partner with Dr Hussain. We are dedicated to providing an effective primary care service to ensure that our patients receive the highest quality of medical care and attention within the National Health Service. Our primary care team consists of the general practitioners supported by our two practice nurses, a healthcare assistant, two phlebotomists, plus a team of district nurses, school nurses, health visitors and a community midwife who are attached to the practice. We are currently part of the Buckinghamshire Primary Care Trust which came into being on 1st October 2006, amalgamating three former PCTs from the Wycombe area, Vale of Aylesbury and Chiltern & South Bucks. We are also part of Bucks Primary Care Collaborative, BPCC, which is an association of 34 local GP practices, working together to improve healthcare services for our 305,000 patients. NHS 111NHS 111 is a free 24 Hour helpline, staffed by a team of highly trained advisers, supported by experienced nurses, who will assess you, provide advice and direct you to the local service that can help you best.
We ask you for personal information so that you can receive the most appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team. Freedom Of Information - Publication SchemeThe Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the ‘classes’ of information the practice intends to routinely make available. This scheme is available from reception.
