If you change your name or move house please tell our reception staff of your new name, address and telephone number, which will make it easier for us to contact you and to keep your records up to date. Do let us know your mobile number as well, if you have one. If you move away from the area you will be required to register with a new General Practitioner as soon as possible.

Any change of name must be accompanied by supporting documentation (marriage certificate for example).
