HOW DO I...
REGISTER?

You will need two pieces of personal identification in order to register, one that confirms your identity, preferably with a photograph, ie passport or photo driving licence, and a second that shows your address and confirms your residency in the practice area (utility bill, pension book for example). The NHS no longer sends out medical cards to patients. All newly registered patients will be asked to complete a simple questionnaire and invited to attend for a new patient examination with our healthcare assistant or one of the practice nurses.

All the partners in the practice take on new patients on a rota basis, but in order to keep list sizes for each GP at a manageable level, registration procedures are subject to change. Requests for registrations with a particular GP will always be considered.
